The Adult Commitment Card

This Commitment Card is filled out individually. Married units will need to submit separate forms for where they plan to serve in 2023. To fill out this form, find the ministry headings you plan to serve under and select where you plan to serve. You can pick multiple ministry opportunities if you feel called to serve in multiple ministry areas.
