The Student Commitment Card

This Commitment Card is filled out individually. If you have multiple students, you will need to fill out multiple forms. We require information from both the student and parent of the student. To fill out this form, find the ministry heading you plan to serve under and select where you plan to serve. You can pick multiple ministry opportunities if you feel called to serve in multiple ministry areas.
